Residential Program

The Challenge Center’s Residential Program provides homes, supervision and support to individuals with developmental disabilities or other special needs, with the primary diagnosis of mental retardation. These homes
are licensed by the State of Wisconsin. Within these residences, services are provided to groups of four to six adults who share a comfortable, nurturing home. Each individual has their own private room. Residences aredesigned to provide care, treatment and other individualized services to persons over the age of eighteen, whoneed support or supervision because they cannot live, or do not wish to live independently.

Objectives

  1. Provide a safe environment for home living.
  2. Provide support and supervision to the resident to live as independently as possible.
  3. Provide meals to the resident.
  4. Provide training to assist individuals in achieving the highest level of independent functioning of which the resident is capable.
  5. Provide individualized services based on the needs each resident.

Provision of Services

Services are provided or arranged based on the individual needs of the resident. Services include health monitoring, coordination of medical services and medication management, financial assistance, general supervision, meals, leisure time activities, community activities, arranging family contacts, and providing/arranging transportation for appointments. Services may also include teaching and providing opportunities for residents to increase skills or minimize natural decline in the area of self care, independent living skills, communication skills, and socialization. Assistance with personal cares is arranged or provided to the resident based on assessments.

Location

This program operates in homes located in the city of Superior, Wisconsin.

Methods

The program is supervised by the Residential Director and assisted in each of the homes by a Team Leader, Assistant to the Team Leader and a number of supportive home care staff. Staff are experienced, trained in a state approved curriculum and have undergone background checks and health screenings.

Eligibility

Placements are usually coordinated with case managers from a County and based on an Individual Service Plan that focuses on the specific needs of the individual. Private Pay may be arranged.
